Providing Advanced Primary care directly to patients and indirectly working with Nurse practitioners and medical assistants and other members of an interdisciplinary team to formulate treatment plans and other care plans.
Participating in community outreach and patient education and prevention programs.
Maintaining patient files, encountering information, correspondence, clinical data, and other information in a confidential and secure manner.
Examining the health of a patient to help make proper diagnoses of clinical conditions and prescribing medication, treatments, and follow-up care as required.
Educating, advising, and empowering patients on preventive healthcare and disease prevention.
Participating in ongoing learning to help formulate innovative, predictive, and preventive diagnostic or wellness programs.
Chronic disease management of the patients with chronic diseases such as Diabetes, Heart failure, COPD, Asthma, chronic kidney disease, Hypertension.
Ability to interact and collaborate respectfully with a variety of people from diverse backgrounds
Excellent interpersonal and communication skills, both written and verbal
Board Certified or eligible for certification in ABMS
Strong drive for improving and working on professional development
Strong organizational skills and ability to multitask
Strong critical-thinking and problem-solving skills
Knowledge of Electronic Medical Records (EMR) systems
Requirements:
Requires a MD or DO.
Current unrestricted medical licensure in applicable state(s) and board certification. If board eligible, must become board certified within 12 months.
DEA license required.
Satisfactory completion of a Tuberculosis test is a requirement for this position.
